Did you try the experiment of using bin/ipxe.usb (on a USB key)?  The 
difference there is that the driver would be starting from a clean slate 
(with no previous driver having touched the card).

That will be some (probably trivial) mistake in the build process. 
There's nothing magical about undionly (versus other builds) when it 
comes to embedded scripts.
