Thanks; that's narrowed it down one stage further.  It looks as though the 
problem happens when iPXE tries to load a PXE stack from the Broadcom 
(14e4:1639) PXE ROM.

Could you try DEBUG=pci,undi,undirom,undiload,undinet,pxeparent ?
