The presence of "/IPv4(0.0.0.0)" as part of the device path is looking 
wrong to me.

Could you boot into Shell_Full.efi (instead of snponly.efi), and post 
the output from

   devtree -d

(for PciRoot(0x0)/Pci(0x1,0x0)/Pci(0x0,0x0) and its child devices), and for

   dh <handle>

for the handles corresponding to 
PciRoot(0x0)/Pci(0x1,0x0)/Pci(0x0,0x0)/MAC(5CF3FC6E4738,0x0) and 
PciRoot(0x0)/Pci(0x1,0x0)/Pci(0x0,0x0)/MAC(5CF3FC6E4738,0x0)/IPv4(0.0.0.0)?
